Profile:

Heather Sakely, PharmD, BCPS, is the Director of Geriatric Pharmacotherapy and the Director of the PGY2 Geriatric Pharmacy Residency at UPMC St. Margaret.   She attended the Duquesne University Mylan School of Pharmacy, with a concentration in Geriatrics, completed a specialty pharmacy residency in Family Medicine at UPMC St. Margaret, and is board-certified in Pharmacotherapy. Dr. Sakely provides patient care as part of an interprofesssionl team in the skilled nursing facility, personal care home and office based geriatric practice.  She leads a team of pharmacists caring for patients across the continuum of geriatric care.  She is the principle investigator of the PIVOTS research group:  an interprofessional team of clinicians and researchers from UPMC St. Margaret, the University of Pittsburgh School of Pharmacy, VA Healthsystem and Presbyterian Senior Care, studying the role of the pharmacist paired with the geriatric team, providing direct patient care at all levels within the healthsystem that older adults reside in and transition through.  Dr. Sakely is faculty within the UPMC St. Margaret Pharmacy Residency Programs, Family Medicine Physician Residency Program, and Geriatric Fellowship.  She is a member of the American Geriatrics Society, American College of Clinical Pharmacy and the American Society of Healthsystems Pharmacists. She sits on the Board of Directors for the Pennsylvania Geriatrics Society — Western Division — and is a contributing editor for the Allegheny County Medical Society Bulletin. Dr. Sakely also serves as an adjunct instructor faculty for the University of Pittsburgh.
